The owners bring expertise and empathy to every aspect of care, providing exceptional support and personalized attention to you and your loved ones.
JACKIE CWIAKALA DIRECTOR OF PATIENT CARE SERVICES/TRANSITIONAL CARE NURSE
With 9 years in home health care and 15 years in nursing, I gained extensive experience, including twelve years at Chicago’s largest medical center specializing in Telemetry, Stepdown, and CVICU.
I hold an MSN in Nursing Education and have served as a Clinical Nursing Professor at DePaul University, teaching in both Master’s Entry and BSN Programs at Dominican University and Resurrection University.
Additionally, I’ve worked as an Adult Inpatient Case Manager, showcasing proficiency in managing the transitional care phase for patients.
